POLICY IS IN USE AND IS PRODUCT IS BEING MARKED WITH THE TIME BUT THE POLICY IS NOT ON FILE. A RISK CONTROL PLAN IS REQUIRED FOR COLD HOLDING DUE TO 2X REPEAT CRITICAL VIOLATIONS FOR COLD HOLDING 3 - 501. 16. THIS WILL BE OBTAINED AT THE TIME OF THE FOLLOW UP INSPECTION. VENTILATION AND INSTALLATION OF A STOVE/OVEN AND FRYER AND TABLE TOP ELECTRIC GRILL, AND DISHMACHINE WITHOUT HEALTH DEPARTMENT APPROVAL. IN ADDITION A DOUBLE DOOR REACH IN COOLER AND SINGLE DOOR REACH IN FREEZER AND HEAT LAMP ABOVE FREEZER WAS INSTALLED.
